Support Connection - Level 1

Support connection helps participants use their NDIS plan effectively.

Support connection helps a participant:

  • Understand their NDIS plan
  • Connect with broader systems of supports
  • Connect with providers

Support Coordination - Level 2

Providing support to navigate the NDIS in all its complexity. Working in-depth with you to understand, prioritise and assist you to implement your NDIS plan. Support Coordination focuses on supporting participants to direct their lives, not just their services. This involves working together to understand the funding, what participants expect from services, and how participants want this designed. Support coordinators also assist participants to build and maintain a network of formal and informal supports.

Specialist Support Coordination - Level 3

Providing specialist assistance when your needs and NDIS plan are more complex. Our Specialist Support Coordinators recognise the importance of the right relationship with you and therefore our team of Allied Health Professionals and Social Workers will work with you to resolve any complicated or crisis situations and implement changes to your plan.

Help participants reduce complexity in their support environment, so they can connect with funded supports as well as broader supports. Negotiate support solutions with multiple stakeholders to achieve well-coordinated plan implementation. Help manage crisis points for participants, ensuring participants have access to relevant supports during a crisis.

Psychosocial Recovery Coaching

Psychosocial Recovery Coaching is a strengths-based, client-centred, and solution-oriented approach that can help individuals who are dealing with the challenges of mental and behavioural health. Psychosocial and Recovery Coaching draws on the best of psychology and coaching and helps people move forward with their lives.

Psychosocial and Recovery Coaching services can help:

  • Identify participants’ goals and develop a plan to achieve them
  • Build on participants’ strengths and find new ways to overcome your challenges
  • Increase participants’ confidence and motivation
  • Learn new skills and strategies
  • Communicate more effectively
  • Manage stress and difficult emotions
  • Navigate the mental health system and access resources
